TEST FOR BILIRUBIN

What is bilirubin?

Bilirubin is a yellowish substance in your blood. It forms after red blood cells break down, and it travels through your liver, gallbladder, and digestive tract before being excreted. 





What are the causative agents of change in bilirubin level (high/low)?

Following are the causative agents of bilirubin;

  • Gallstones
  • Gilbert’s Syndrome
  • Liver dysfunction
  • Hepatitis
  • Bile duct inflammation
  • Intrahepatic cholestasis of pregnancy
  • Hemolytic anemia

Biochemistry test for bilirubin detection;

For detection of level of bilirubin in body 3 test are commonly used;

  1. Total bilirubin
  2. Direct bilirubin
  3. Indirect bilirubin

Direct bilirubin:

In direct bilirubin simply reagents are used & then analyzed by using biochemistry analyzer. The procedure is;

  • Add 200ul serum in test tubes labelled as TEST & BLANK.
  • Add 200ul reagent-1 from bilirubin regent kit, in both test tube
  • Add 50ul reagent-2 from bilirubin regent kit, in test tube labelled as test only.
  • Keep at room temperature for 10 min, in both test tube
  • Add 1000ul reagent-3 from bilirubin regent kit, in both test tube
  • Keep at room temperature for 5 min
  • Add 1000ul regent-4 from bilirubin regent kit, in both test tube
  • Keep at room temperature for 5 min & then sip them in biochemistry analyzer 
  • blank test tube should be sipped in blank & same is for test labelled test tube that is sipped in sample

Indirect bilirubin:

In indirect bilirubin simply reagents are used & then analyzed by using biochemistry analyzer. The procedure is;

  • Add 200ul serum in test tubes labelled as TEST & BLANK.
  • Add 200ul reagent-1 from bilirubin regent kit, in both test tube
  • Add 50ul reagent-2 from bilirubin regent kit, in test tube labelled as test only.
  • Keep at room temperature for 10 min, in both test tube
  • Add 2000ul normal saline in both test tubes
  • Keep at room temperature for 5 min & then sip them in biochemistry analyzer 
  • blank test tube should be sipped in blank & same is for test labelled test tube that is sipped in sample

Total bilirubin:

It is sum of both direct & indirect bilirubin.
